Yeah, if you have been taking even just 40mg a day you are going to get dope sick without a doubt. And it's not fun at all!! You wouldn't be able to go to work for a few days that's for sure. I'm a little surprised that you had a doctor give you opiates with tramadol. I always thought that was a bad combo that could cause seizures.
If you just reduce the dosage you are taking slowly you can get off them without many problems. Instead of taking a whole pill take a half for a week or so, then reduce how often you take them. If you have been taking them every 4 hours start off by moving it to 6,and then 8 and so on. If you start to feel sick just stay on the level you reduced to another week or so. You'll know when you are start to get off them. It really isn't as big of a deal as people make it out to be if you just dose yourself down. I've been through it before, a few times. If you went to the doctor and told him what you wrote here he would do the same thing. He would put you on a weening down program.
If you are serious about wanting to get off them I would talk to your doctor. He can give you something like klonapin that would really help a lot! Doctors know very well that some patients develope a abuse problem. But that just part of dealing with chronic pain. He will help you get off them without you having to go through withdrawls. It's a 7-10 day hell you don't want to put yourself through if you don't have to. I know it has kept me from abusing them for several years. I wouldn't ever go through them again.
You are going to start to have a lot of problems sleeping. If you are over 21 try a glass of wine before bedtime. It really helps. If you live in a state that has medical marijuana you may want to look into it. It will help you with getting off the opiates and help you with the pain. I live in California so it's not a problem for me. I've been getting a Rx for medical pot for several years. Without it I would be taking far more pain medications then I still have to do for a spine injury. I think it is medically legal in 12 states now. I'm not saying go out and smoke pot, but if you live in a state that it is available it would worth looking into. Many people that have legit medical conditions use it instead of opiates. It's a shame that states like California have kind of abused the law. It use to be you had to have a condition like cancer, MS or a spine injury to get it. Now it's about anybody that can pay $150 and have a doc write a letter. It actually has made it harder on those that do have serious chronic pain. And by the way, I'm not a kid,, I'm almost 60.
or your other choice is go to a methadone clinic and get on the 14 day detox program. But don't get caught up in the maintenance program. Stick with the 14 day and you will be able to kick the opiates without barely any withdrawls at all. The only thing you have to do to go through it is to test dirty for opiates and they will help you. And once again you are going to have sleep problems. The wine or Tylenol PM help but not like klonopin or valium would.
I've done it both ways and it's not that hard if you really want off pain meds.
But you are going to have to find a way to deal with the pain you have been getting the pain meds for. You might want to see if there are any pain management clinics in your area.

When you do kick you should be proud of yourself. You understood the problem that was happening and took action to correct it. A lot of people just give in and keep taking more and more until they have to go through withdarawls or get in trouble for doctor shopping. It really doesn't have to be that bad for you. You will have a bigger problem trying to control the pain. Good luck
