    The nurse is providing discharfe instructions to an elderly client taking the following oral medications: warfarin 5 mg., furosemide 20 mg daily, digoxin 25 mg qod. Which of the following is most important to include in the instructions?

    A. "You cant take the pills with meals"
    B. Call the doctor if you notice any bruising
    C Use a pill box to remind you of the medicines you need to take
    D take the furosemid in the morning so that you can sleep tonight

    It's a tough call, but I would say B is probably the most important as it is
    a) not necessarily obvious that this needs to be looked out for
    b) potentially very serious if ignored

    PATIENT, NOT CLIENT!!!

    "Patient" is not a dirty word. Lawyers have clients. Accountants have clients. Doctors and nurses have patients. It's OK to say it.

    BTW, each of those instructions is important. I don't know how (or why you would want to) make any of this information more important than the rest of it.

    digoxin 25 mg qod -Take with water, do not stop med without speaking to health care provider for conditions may worsen.
    furosemide 20 mg daily - Drink Plenty of water, tell health care provide if you have trouble urinating, do not take longer then prescribed and test blood with this medication to make sure levels are normal.
    warfarin 5mgs. - alert bracelet stating that you are taking warfarin, in case of emergency, careful not to eat certain foods, or ingest too much vitamin K, do not have anything w/ cranberries. Avoid dieting. Drink glass of water.

    Personally, A would be the common instruction for the above-mentioned medication as food affects the said drugs' absorption & bioavailability.
