Differential diagnosis (diseases with similar symptoms/ presentations) for IBS-D includes: colonic cancer, inflammatory bowel disease, Coeliac disease (wherein the body's immune system, triggered by gluten, attacks its own tissues), gastroenteritis, ischaemic colitis, gynaecological problems (eg endometriosis, ovarian tumours), anxiety with or without depression, somatization (Briquet's disorder or, in antiquity, hysteria) and panic disorders.
