Go to the California board of Medical Examiners and put in his name. This department or a version of it will have his license number and contact details. You can also contact the pharmacy you used there and they can mail you your prescription history.

Just go to Pharmacy and ask for what is called a "Pharmacy Profile"...They will print and give it to you...Good Luck..

Before I got my referral to a PM the first time. I walked into walgreens and asked for a printout of my script history for a year. They did it right there, no charge.

I also had to walk into my old doc and physically get my records from him which wasn't as easy. They never forwarded anything, I owed money. Had to make a little bit of a scene, but once they started getting embarassed infront of all their patients, they went and got them in a minute.

I've never had trouble getting my records. I just called again the other day to an ortho I had seen and asked for my records. They belong to you so they shouldn't hesitate to give them to you. I like to have them for second opinions, or my disability insurance.
    Hope you locate them.

BSI were supposed to forward them to my specialist and then PM. They never did after 3 months and 10 phone calls. So I had to walk my rear in there a make a fuss. I went to 2 PM appointments without them having any of my records.

I ask my doctors for a copy of my medical records since my last visit. Not the entire file, just the information placed in the file since my last visit. I keep them in a binder.

Hindsight is a wonderful thing but I do recommend everybody ask for a copy of their records ~ it's a common request ~ and start to create your own file.

I never even saw my MRIs or X-rays until I got them in my hands after requesting the records. It was nice to at least look at them after all the $$ I shelled out for them.

I was told the results by the doctors, but it woulda been nice to have them actually show me where the problem was. Also the radiologists reports said different/more things than the doctors told me.

It is a good thing to have. If I'm feeling upset with my old docs, I can walk into a new one with everything he needs to see. Instead of making a bunch of phone calls to 3 different places requesting faxes/emails and CDs. And then waiting for it all to happen weeks later.

if you want your medical records you need to download a HIPPA Compliant authorization. Google it. Fill it out and send or deliver it to your doctor. In NY State the max they can charge you is $.75/page for photocopying. Included will be your MRI reports etc. It would be rare that someone would need an actual copy of your MRI film. In that case you would have to do the same HIPPA and wait for a bill to copy it (its on special paper and can be a little expensive.)

If you don't have insurance and pay for an MRI...
They should give you it on CD when you leave.

@Pain What is a pharmacy profile and would most doctors accept this as a record? Does it list why you got he meds or is it just a list?

Some pharmacies like Walgreen's allow you to access your prescription history online, but you must electronically verify your identity by answering multiple choice questions such as, "Which one of the streets have you not lived on?"

It seems to be really easy to get a pharmacy record and most do it for free. Getting records from your physician was damn near impossible for me when I decided to switch doctors. Now at every office visit I ask for a copy of my treatment plan just in case.
