What disorder is being treated by the warfarin therapy? Are you taking any other medications aside from warfarin? Were these words legibly written? Are these the only words written in the chart? Is this how it was exactly written, i.e. all caps?

The most logical meaning for these words is: for warfarin therapy, take 1 each, for minor stroke as ordered?

The only person most likely to know what this means is the person who wrote it since he knows the medical history and other circumstances pertaining to this specific warfarin therapy, as well as is familiar with his own shortcuts/ codes.

If you give more details or background on it, it would be easier to decode this warfarin therapy order. Hope this helps!
